What is the presentation of wegeners granulomatosis in the kidneys?
Focal necrotising glomerulonephritis with crescents
What is the difference betwen microscopic polyarteritis and wegeners?
Wegeners affects respiratory tract, kidneys and other systems in the body whereas microscopic polyarteritis is more localised to the renal system
What is an important diagnostic test in vasculitis?
Immunostaining of P-ANCA and C-ANCA
Or
ELISA for anti-myeloperoxidase (C-ANCA) and Porteinase 3 (P-ANCA)
Which anti body corrolates to wegeners and which one to microscopic polyarteritis?
Wegeners = C-ANCA M.P = P-ANCA
What is the downfall of ANCA testing in immunostaining?
False positive in conditions such as IBD
What kidney condition can infective endocarditis cause?
Glomerulonephritis +/- small vessel vasculitis
In infective endocarditis, what would suggest that there is renal involvement?
Abnormal urea/creatinine
Haematuria, red cell casts
Reduced complement levels
What is multiple myeloma?
Monoclonal proliferation of plasma cells in bone marrow producing an excess of immunoglobulins and light chains
