RED : means
Emergent, immediate
= life threatenign injuries , treatable, minnimum people and supplies and time to save them
= lke a STEMI
Yellow means
Urgent
= has some more time without significant mortalitly rate
Green means
Delayed : pt could go to primary care however did not find chance (DONT require ICU)
BLUE means
Expected : pt with injuries requiring extensive tx that exceeds medical recources available
BLACK means
Pt comes in dead
RED or YELLOW pt what do you do first
- ABCDEs (Airway, Breathing, circulation, Disability / neural, Exposure)
- Then do the HPIs
LEVEL 1 trauma
Highest level of care = in house surgical care 24/7 = 5000 trauma tx per year = burn units = can take care of all body parts and all specialties available
LEVEL 2 trauma
= same as level 1 however dont have some specialties
= not needed to do as many pts per year
LEVEL 3 trauma
= ER and some stability and care
= JOB is to stabilize and then transfer to level 2 or 3
Avoid anchoring bias
Beliving 1st thing you hear (someone says most likely this and then you only look at that)
New onset afib what sould you think about
Stoke , since that is a high risk
Holo sign
Ruptured TM with CSF and blood coming out (from a leak inside brain)
Pnumosephallys
Air inside skull = can happen in meningitis (abnormal) = neurosurgen should evaluate
Order problems
CC first then manage the chronic illnesses
GOOD THING TO KNOW IN AN INSTITUTION OR HOSPITAL
LEVELS OF CARE (where to send a pt after stabilization like general medicine floor ICU or telemetry, and services available like IM hospitilist ICU)
Serious pt vs critical pt
SERIOUS : vitals may be unstable, acutely ill and indicators are questionable
CRITICAL : vitals areunstable, acutely ill and indicators are unfavorable
When should a pt go to ICU based on vitals
If vitals need to be checked more then 1 time in 2 hours
When doing labs what is a good thing to think about
Consolodate all of them (do them at the same time) = helps everyone
PPE for contact pt
Gloves and apron
PPE for droplet pt
Gloves, apron, surgical mask , N95 mask if AGP (airsoliced Procedure)
PPE for airborn pt
N95 mask, apron, gloves
How much Na is in normal saline
154 mEq/L (normal level is 130)9
Medications : how to give and document
SAY FOR WHAT : THEN Say what medication , howlong, and route of administration , how often , ,pain scale
(Acetaminophen 500mg q6h PO prn pain 1-3)
(Tramadol 50 mg q6h PO prn pain 4-6)
(Oxycodone 5mg q4h PO prn pain 7-10)
