What is it?
Systematic gathering of data (past and present) as to why the pt. was PT services.
HISTORY
How is it gathered?
Individual, Family, Significant others. Caregivers, Others (counselor, teacher, case managers, employer.) Individual's record.
REVIEW OF SYSTEMS
What are the 8 systems?
Cardiovascular/Pulmonary system. Endocrine. Head, Eyes, Ears, Nose, Throat. Gastrointestinal. Genitourinary/Reproductive. Hematologic/Lymphatic. Integumentary. Neurologic/Musculoskeletal.
SYSTEMS REVIEW
What is it?
Brief examination of
- anatomical and physiological status of the 4 scope of practice systems
- communication ability, affect, cognition, language, and learning style .
Tests and Measures are used to do what?
Rule in or Rule out causes of impairment, activity limitations, and participation restrictions.

What is a PT diagnosis?
PT Diagnosis–based on what is happening within the mvt system.
PTs use labels that identify the IMPACT of a condition on function at the level of the system (especially the mvt system) and at the level of the whole person.
Describe in terms of ROM, strength, and not w a particular pathology.
A PT Diagnosis is also called a Differential Diagnosis. Define.
Integrating and evaluating data obtained during the examination to describe the individual’s condition in order to determine the prognosis and plan of care.
