- Custom Privilege: If an application has requested a Custom Privilege.
- Declined Application for 1 or more Facilities: If an application has any declined facilities.
- Gaps: If an application has been submitted with any gaps in education, hospital, and other for longer than 6 months.
- Missing Required Fields/Steps: If any exemptions are requested for required fields/sections. (ALWAYS ENABLED)
- Negative Attestation Questions: If an application is submitted with any negative attestation questions.
- New Privilege Requested: If an application has a new privilege requested.
- No Privilege Selected on form: If an application is submitted with no privileges selected on the form.
- Previous Rejection Message: If an application has been previously returned.
- Privilege Not Renewed: If an application has a privilege not being renewed.
- Required Attestation Question Missing: If an application is submitted with any attestation questions missing.

What are MD-App Documents?
General documents the MSO uses to attach the an application that is signed using DocuSign and AdobeSign; doesn’t always need a signature. These documents will then be merged into the provider’s profile
What is the template document map feature?
Allows you to designate specific PDF files that automatically merge with provider data when it’s imported from the MD-App application. It’s usually used with State-mandated applications but can be used for any pdf template.

What are Signable Merge Templates?
PDF templates are signed within MD-App using DocuSign and AdobeSign for state mandated applications. When the provider’s application is imported into MD-Staff the state mandated application will appear in the files folder along with any other documents that required signatures and a copy of the MD-App application. MADE BY ASM.
A Provider resigned from staff several years ago and wants to reapply. What workflow should be followed to send this Provider a new application?
- Unselect the ‘Archived’ Checkbox.
- Credentialing > MD-App > Begin a Reappointment.
BUT you should be choosing an Initial Application template that has been created and is allowed to be used as a Reappointment.
What are ‘Required Objects’ in MD-App?
They are the minimum entries/sections in the application that must be filled out in order to submit the application, unless they submit an exemption for it.
