PATHOLOGY: Testicular Torsion

A
  • Intravaginal: rotation w/i tunica vaginalis
    • Usually peripubertal
  • Extravaginal: rotation on spermatic cord
    • Usually neonates

CLINICAL FEATURES: Testicular Torsion

A

Symptoms

  • acute onset, severe pain
  • pain radiates into iliac fossa
  • Nausea + vomiting

Signs

  • Tender swollen testis
  • Erythema overlying scrotal skin
  • Cremasteric reflex absent on affected side* (bad sign)
    • (Retraction of testicle when you stroke thigh - same side)

INVESTIGATIONS: Testicular torsion

A
  • Doppler Ultrasound Scan
    • Flow of testicular blood
  • Scrotal Exploration must be down to rule out torsion

MANAGEMENT: Testicular Torsion

A

MEDICAL EMERGENCY

SURVIVAL BEST IF TREATED w/i 6 HOURS

  • Surgical untwist testes
  • Viable testes = fixation
  • Necrotic testes = orchidectomy
