PATHOLOGY: Testicular Torsion

Intravaginal: rotation w/i tunica vaginalis
- Usually peripubertal

Extravaginal: rotation on spermatic cord
- Usually neonates

CLINICAL FEATURES: Testicular Torsion
A
Symptoms
- acute onset, severe pain
- pain radiates into iliac fossa
- Nausea + vomiting
Signs
- Tender swollen testis
- Erythema overlying scrotal skin
- Cremasteric reflex absent on affected side* (bad sign)
- (Retraction of testicle when you stroke thigh - same side)

INVESTIGATIONS: Testicular torsion
A
- Doppler Ultrasound Scan
- Flow of testicular blood
- Scrotal Exploration must be down to rule out torsion

MANAGEMENT: Testicular Torsion
A
MEDICAL EMERGENCY
SURVIVAL BEST IF TREATED w/i 6 HOURS
- Surgical untwist testes
- Viable testes = fixation
- Necrotic testes = orchidectomy
