PATHOLOGY: Prostatitis
A
- Experienced by 50% of men in lifetime
- Category 1:* acute infection of prostate (E. Coli, Pseudomonas, Klebsiella)
- Category 2:* chronic infection of prostate (E. Coli, Pseudomonas, Klebsiella)
- Category 3:* chronic pelvic pain syndrome (w/o infection but w/ inflammation)
- Category 4:* Asymptomatic inflammatory prostatitis (incidental finding after TURP/biopsy)

CLINICAL FEATURES: Prostatitis
A
Symptoms:
- Pelvic, urethral, perineal or rectal pain
- Frequency, urgency
NB: if category 1: may be urinary retention and septicaemia
- Signs*
- PR = tender, boggy prostate

INVESTIGATIONS: Prostatitis
A
- Cultures*
- MSU = pyuria
Diagnosis confirmed by prostatic massage and microscopy with culture of prostatic secretions

MANAGEMENT: Prostatitis
A
If infected
- Ciprofloxacin/ofloxacin treatment
- TURP if abcesss
- Retention treated with suprapubic catheterisation
If category 3 - difficult to treat
- alpha-blocker
- NSAIDs
- Diazepam
- Prostatic massage
- Lifest6yle: no caffeine/alcohol/spicy foods
