What are the 4 ways infants lose heat?
Convection - Air takes heat with it
Evaporation - water takes heat with it when it leaves
Conduction - a surface can transfer heat to or from
Radiation - The baby radiates heat- keep the room warm and the baby away from cold walls
How do babies keep warm and why do we care?
Brown fat, not with shivering
Because if they can’t keep warm they get hypoglycemic (shaking twitchy), and respiratory distress (grunting, retractions, nasal flaring)
Normal temperature for a neonate is
36.5-37.5
If a neonate uses its brown fat up:
It uses up glycogen instead
Increases use of oxygen- meatbolic acidosis
Decreases production of surfactant ->
Reverts to fetal circulation

How do you recognize pathologic jaundice?
Physiologic and Pathologic
Physiologic is common, mild, transient. The infant has an immature liver and it can’t handle the breakdown of all the blood cells. Peaks around day 4-5 and will be gone in two weeks. Never happens the first day.
Pathologic happens the first day of life, because baby and mom’s blood are incompatable and the baby’s blood is breaking down.
What is kernicterus?
Unconjugated bilirubin crossing the blood brain barrier and causing permanent brain damage
What are the newborn reflexes we do in a neonatal assessment?
Moro- the arms out response to dropping
Sucking - they suck on your pinky
Rooting - turn toward stroked cheek
Babinksy - their toes splay when you stroke the foot
Stepping - they slowly take steps when held to the ground
Palmar/Plantar - they grasp whatever is placed in palm or near toes
Fencing- turn their head, their arm goes out
