What is the Sacroiliac Joint (SI Joint)?

the spine and the lower extremity
What are the functions/responsibilities of the sacrum?
- Provides stability
- Transmits the weight of the body from the vertebral column to the pelvic region

What are the functions of the SIJ?
- shock absorption
- transfer of bodyweight between the vertebral column and the lower extremity
What are the movements of the SIJ?
nutation and counternutation
Nutation
when the sacrum moves forward/anteriorly and down
Counternutation
when the sacrum moves backward/posteriorly and up
Triad for SIJ diagnosis
(+) history
(-) BCE for lumbar spine
(+) provocation test(s)
SIJ provocation tests
- Dorsolateral
- Ventromedial
- Thigh Thrust (Ostgaard)
- Sacral Thrust
- Gaenslen
Dorsolateral test
patient = supine
examiner = either side
- cross arms and place on both ASIS
- apply a dorsolateral force to the ASIS
- hold for 3 mins unless they feel pain before the 3 mins; if so terminate the test
What is a positive dorsolateral test?
if unilateral pain is provoked in the abdomen, buttock or the leg
Ventromedial test
patient: side lying with affected area up
examiner: behind patient
- compress the iliac crest by applying a ventromedial force
- hold for 3 minutes unless patient experiences pain prior to the 3 min
What is a positive ventromedial test?
reproduction of symptoms
Ostgaard (Thigh Thrust)
patient = supine
examiner = opposite side
- Flex the patient’s hip to 90 degrees
- Place your hand on the
patient’s sacrum just medial to the PSIS on the test side - Apply an axial force through the femur
- If the first side is not painful the other side should be tested in the same manner
