excoriation
traumatic lesion breaking the epidermis and causing a linear red mark (i.e., a deep scratch); often self inflicted
lichenification
thickened and rough skin characterized by prominent skin markings; usually the result of repeated rubbing
macule
flat, circumscribed area, 5mm it is a patch
papule
elevated or dome- or flat-top lesion 5mm diameter it is a nodule
plaque
elevated flat-top lesion, usually >5mm diameter
pustule
discrete, pus-filled raised lesion
scale
dry, horny, platelike excrescence; usually result of imperfect cornification
vesicle
fluid-filled raised area 5mm is referred to as a bulla (blister refers to both)
acantholysis
loss of intercellular adhesions of keratinocytes
acanthosis
diffuse epidermal hyperplasia
dyskeratosis
abnormal keratinization occurring prematurely within individual cells or groups of cells below the stratum granulosum
hyperkeratosis
hyperplasia of stratum corneum, often associated with an abnormal amount of keratin
lintiginous
linear melanocyte proliferation along the epidermal basal layer; can occur as a reactive change or a melanocyte neoplasm
papillomatosis
surface elevation caused by hyperplasia and enlargement of dermal papillae
parakeratosis
keratinization characterized by retention of nuclei in stratum corneum; normal on squamous mucosal membranes
