NEUROCHEMISTRY: enhances DA and NE. no 5HT
.
ADVANTAGES: 
no sexual side effects
good for depressed Parkinson's pts (boosts DA)
no weight gain
no P450 interactions
reduces nicotine craving

DISADVANTAGES:
increased seizure risk (DA)
doesn’t work for panic disorder or OCD
contraindicated in bulimics or history of seizures

NEUROCHEMISTRY: at higher doses blocks NE (and maybe DA) reuptake. DO NOT combine with MAOI's!
ADV:
little to no weight gain
minimal sedation
no P450 interactions
higher dose will have more effect
DISADV:
has all 5HT side effects
increases BP

NEUROCHEMISTRY: at starting doses blocks reuptake of NE AND 5HT. do not combine with MAOI’s! interacts with P450

ADV: better for depressed chronic pain pts

DISADV: all 5HT side effects, some anticholinergic side effects, interacts w/ P450

Mirtazapine

dual mechanism agent

A

NEUROCHEMISTRY: 1) blocks alpha-2 -> disinhibit 5HT
2) blocks 5HT 2 and 3 receptors -> minimize serotonin side effects
ADV: less sexual side effects
no P450 interactions
DISADV: sedating, weight gain, more side effects at lower doses

side effects of MAOI’s

  1. HYPERTENSIVE CRISIS: from increased NE and also more tyramine from fermented foods can enter blood
    .
  2. HYPERMETABOLIC CRISIS (serotonin syndrome): fever, NM irritability, altered consciousness -> coma, seizures, possible death. caused by giving MAOI w/ 5HT level raising med (common cold meds, ecstacy, ALL SSRI’s)
    .
  3. all side effects of blocked ACh, alpha 1, and histamine receptors

lowers suicide rates

cons:
narrow therapeutic window. must moniter blood levels closely! avoid dehydration and kidney disease/reduced function
less likely to help “rapid cycling” bipolar pts (>4 episodes/year)

side effects: upset GI, termors, weight gain, increased thirst and urination, hypothyroidism, delayed cardiac conduction, kidney abnormalities
