A Dx of DM is made if the subject has: A “casual” plasma glucose conc. of
≥ 200 mg/dl, plus symptoms of diabetes, e.g., polyuria, polydipsia, weight loss (i.e., the classic symptoms). [“Casual” means any time of day without regard to the time of the last meal.]
A Dx of DM is made if the subject has: Fasting plasma glucose [FPG] conc. of
≥ 126 mg/dl (7.0 mM). [“Fasting” means no caloric intake for ≥ 8 hours.]
A Dx of DM is made if the subject has: A 2-hr plasma glucose [PG] conc. of
≥ 200 mg/dl (11.1 mM) in a standard oral glucose tolerance test [OGTT], using a dose of 75 grams of glucose.
A Dx of DM is made if the subject has: Hb A1c : ≥
Hb A1c : ≥ 6.5% (blood taken anytime; fasting not required; acute illness & stress have only small effects)
The OGTT is not recommended for routine clinical use. Unless?
he subject has unequivocal hyperglycemia with acute metabolic decompensation, these criteria should be confirmed by repeat testing on a different day.

If the 2 hr PG is ≥ 140 and < 200 mg/dl, the Dx is “Impaired Glucose Tolerance” (IGT).

24-28 weeks, if +, then a “diagnostic test” is performed. However, certain patients are considered so “at risk” that the screening test is skipped, going straight to the “Dx test”.

Do not need screening. But to be in this class, they must have all of the following: <25 y, normal wt., no first degree relative with DM, and not Hispanic, African-American, Asian, native American, or Pacific Islander, and no family history of diabetes.
