What is a closed head injury?
- Result of a blow to the head or a sudden violent mtion
- Can be diffuse (all over brain) or focal (one spot in brain)
- Most common head injury
Causes of TBIs
- Falls ( 28% rate highest for 0-4 y.o.)
- Motor vehicle (20% highest among 15-19)
- Struck by/against events (19%) (sports and rec. colliding w/ a moving object or stationary object
- Assaults (11% firearm use is the leading cause of death relate to TBI)
What is an open head injury?
-Physical damage to the brain from an outside source, object breaks the skull and enters the brain.
Symptoms of a TBI
- loss of consciousness
- dilated pupils
- respiratory distress
- convulsions
- headache
- dizziness
- nausea and vomiting
- cerebrospinal fluid leaking from nose or ears
- speech and language problems
- vision issues
- emotional and behavioral changes
What are the Rancho Los Amigos levels of cog. functioning scale
I. no response
II. General response (startle movt.)
III.Localized response (can do a command)
IV. Confused agitated (mad, screaming)
V. confused inappropriate (don’t know where theyare, inappropriate behavior.)
VI. Confused appropriate (not sure, not mean)
VII.Automatic appropriate (knows routines but can get off task easy)
VIII.Purposeful appropriate w/ standby assist
IX. Purposeful appropriate w/ standby assist on request
X. Purposeful appropriate, modified independence
What does the acute phase of TBI consist of?
Severity of injury impacts clinical signs
Focus is on saving life
OT involved in PROM, positioning, splinting
What does the rehabilitative phase of a TBI consist of?
Medically stable
focus is on assessment
OT provides intervention (ADLs, play and leisure, organize and management skills)
