Isoniazid HCl
first line drug for active TB, used in combination. activated by catalase peroxidase, and targets the inhA gene product found in the cell wall
What are the resistance mechanisms for isoniazid HCl?
Mutations in the KatGgene and mutations in the inhA gene
What are the AE of isoniazid?
hepatotoxicity, neurotoxicity, hypersensitivyt reactions
ethambutol
first line TB therapy, helps inhibit resistance to other drugs. inhibits TB arabinosul transferase encoded by embB gene
What are the AE for ethanbutol?
optic neuritis, peripheral neuropathy
Pyrazinamide
first line TB drug, used for the 1st two months of therapy.
Streptomycin
second line TB drugs
Multidrug resistant TB
resistant to both INH and refampin
If tb is rifampin resistant, how is treatment different?
No 6 course therapy available, must have therapy for 18-24 months
What is the 6 month protocol?
RIPE: rifampin, INH, pZA, ethambutol
