Patient presents with chronic cough and night sweats. Sputum culutre grows acid-fast bacilli that stain red with Zeihl-Neelsen staining. What is the infection?
TB
TB vaccination?
BCG vaccine- live attenuated mycobacterium bovis
Prior have mantoux test
TB presentation?
Chronic, gradually worsening
* Cough
* Haemoptysis
* Lethargy
* Fever or night sweats
* Weight loss
* Lymphadenopathy
Chest X-ray of primary TB?
- Patchy consolidation
- Pleural effusions
- Hilar lymphadenopathy
Chest x-ray of Reactivated TB?
Patchy or nodular consolidation with cavitation
Treatment of latent TB?
Isoniazid or rifampicin for 3 months
OR
Isoniazid for 6 months
Treatment for active TB infection?
R- Rifampicin 6 months
I- Isoniazid 6 months
P- Pyrazinamide 2 months
E- Ethambutol 2 months
Needs to be perscribed alongside pyridoxine (vit B6)
Rifampicin side effect?
Red/orange secretions e.g. urine and tears
Isoniazid side effects?
Peripheral neuropathy- numbness/unusual sensations in feet
Ethambutol side effects?
Can cause colour blindness and reduced visual acuity
