2: SFA stenosis 10-20; CTO 5-10 not flush; popliteal stenosis < 2 not tibial
3: SFA stenosis > 20; CTO 10-20 not flush; any SFA flush; popliteal stenosis 2-5 not flush
4: SFA CTO > 20; popliteal stenosis > 5; any into tibial; any popliteal
CTO MODIFIER: significant calcium add 1 grade

Toe pressure: >60, 40, 30, <30
Foot infection:
0: none
1: skin, subcu
2: deeper past subcu
3: systemic
