What is the genetic influence ?
- Consistent association of periodontal manifestations with certain genetically transmitted traits
- 50% of enhanced risk for severe disease by heredity alone
What are the genetic factors?
⚫ Act on and modify host response to bacterial challenge
⚫ Major determinants of susceptibility
⚫ Determinants of rates and extent of disease progression
What are the evidence for genetic control?
- Genetic studies of families with aggressive periodontitis
- Certain chromosomes or loci linked to enhanced susceptibility
- Most rapid progression seen at young age suggesting genetic susceptibility
What are interleukin?
Cytokine released from inflammatory cells
What is genotype?
- Identified as a specific genetic marker
- Places individuals at risk for periodontitis
- Occurs in ~ 30% of population
- 67% of patients with severe periodontitis have IL-1 genotype
- IL-1gene positive patient is 7 times more likely to develop or have advanced periodontal disease
What is the overproduction of interleukin-1 cytokines?
⚫ Does not cause periodontal disease
⚫ Increased host response to presence of bacteria
⚫ Increased inflammatory response
⚫ Increased levels of IL-1 in GCF
⚫ Increased bleeding on probing
What are some neutrophil abnormalities?
⚫ Hereditary deficiencies in neutrophil function or cell numbers
⚫ Can lead to systemic bacterial infections
⚫ Associated with increased susceptibility to severe periodontal destruction

Down syndrome

What is down syndrome?
- Chromosomal abnormailty-trisomy 21
- Abnormal neutrophil function in 50% of patients
- severe agressive disease
What is Leukocyte adhesion Defieciency ?
- Inherited disorder
- Require marrow transplant to survive
- Deficiency in neutrophils prevents adhesion to vessel walls
- Early loss of teeth, severe bone and attachment loss, ulceration and necrosis
What is papillon-lefevre syndrome?
- group of skin disease characterized by hyperkeratotic skin lesions
- marked destruction of periodontium of primary and secondary dentitions
What is Chediak- Higashi syndrome
- Inherited disorder
- recuurent bacterial infection
- rapidly destructive periodontitis
- do not respond to periodontal therapy
- premature loss of both dentitions
What is familial or cyclic neutropenia?
- Decrease in circulating neutrophils
- Severe ulcerative gingivitis/ gingivostomatitis
- Destructive generalized periodontal lesions
What is Ehlers danlos syndrome?
- Inherited connective tissue disorder
- Collagen synthesis defect
- Increased elasticity
- Type VIII: Generalized Periodontitis, Tooth loss between puberty; before 30 yrs
