Surgical nursing Flashcards

1
Q

What post-operative care is required for a GDV patient?

Q

What is the function of the drill bits?

A

Glide hole - drilled using a drill bit that has the same EXTERNAL DIAMETER as the screw being placed
Pilot hole - drilled using drill bit that has same diameter as CORE of screw

Q

What is the function of the countersink?

A

Cuts an indent to allow the head of the screw to be flush with the surface of the bone

Q

What is the functions of the following pins:

  1. Steinmanns pins
  2. Arthrodesis wire
  3. Kirschner wire
  4. Rush pins
A
  1. Similar function as K-wire (pin bones) but larger in diameter
  2. Hold together small pieces of bone too small to be fixed with wire
  3. Pin bones
  4. Pin bones

Q

Discuss how to prepare a patient of opthalmic surgery (7 marks)

A
  1. Wear non-sterile gloves
  2. Apply barrier gel in the eye to protect from clipping
  3. Clip area (roughly 2cm diameter)
  4. Remove clipped hair
  5. Scrub using povidine-iodine (not scrub as can irritate cornea)
  6. Clean surrounding skin with 1:10 dilution of iodine and water for minimum 5 minutes
  7. Flush the eye in sterile saline and iodine 1:50 dilution ensuring all barrier gel removed

11
Q

Outline the wicking effect and risks

A

Occurs when fluid is drawn into the suture material from the environment and surrounding tissues. It is likely to contain bacteria and may cause surgical site infection therefore delaying wound healing

13
Q

What are the 3 types of needles and their uses?

A
  1. Round-bodied = Separate tissue fibres, used in friable tissue such as internal organs
  2. Taper point = Gradually broadens from the tip for greater separation of tissues
  3. Cutting = Cutting through tissue, used for dense tissue eg skin

16
Q

Disinfection

A

Process of destroying microorganisms but not always spores

17
Q

Asepsis

A

Absense of pathogens and their toxins

18
Q

Antisepsis

A

Process of eliminating microorganisms using an antiseptic

19
Q

Sterilisation

A

Destruction of microorganisms & their spores

20
Q

Compare aqueous scrubs to alcohol rubs

A
AQUEOUS SCRUBS:
-5 min scrub recommended
Chlorhexidine Gluconate:
-Significant residual effect
-Can cause skin irritation
Iodine:
-Little/no residual effect
-Reduces chance of skin iirritation
-Causes discolouration of skin
ALCOHOL RUBS:
-90 second contact time (for Sterillium)
STERILLIUM:
-Quick & effective
-Requires pre-scrub
-Significant residual effect
21
Q

Antiseptic

A

Chemical NORMALLY used on living tissues to destroy microorganisms (not specific to pathogenic)

22
Q

What are the 2 types of catgut available?

A

Plain and Chromic

23
Q

What are the tensile strengths of the 2 types of catgut?

A
Plain = 10-14 days
Chromic = 21-28 days
24
Q

Identify 3 other properties of catgut?

A
  • Natural
  • Moderate to high tissue reaction
  • Digested by enzymes
