A
History of ischemic heart disease, CHF, CVA, DM, CKD
High risk surgery -> suprainguinal vascular, intraperitoneal, intrathoracic
3 or more predictors = 11% risk of cardiac death, nonfatal MI, or nonfatal cardiac arrest

A
Known CAD, PAD, or CVD

A
Dyspnea of unknown origin
Current or prior CHF w/worsening dyspnea or change in clinical status if no echo w/in 1 year

A
Unstable or severe angina Recent MI AV block Symptomatic ventricular arrhythmia Supraventricular arrhythmias with uncontrolled rate Symptomatic bradycardia New ventricular tachycardia Severe aortic or mitral stenosis 1-2 risk factors and poor functional capacity
