Aetiology
Dopamine receptor subtype (DRD2) reward system
Substance use disorder
1) impaired control: taking more or for longer than intended, unsuccessful effort to stop or cut down, spending great deal of time obtaining, using, craving for substance
2) social impairment: failure to fulfil major obligations due to use, continued use despite problem caused or exacerbated by use, important activity given up or reduced because of substance use
3) risky use: recurrent use in hazardous situation, continued use despite physical or psychological problem,
4) pharmacological dependence: tolerance to effects of the substance, withdrawal symptoms when not using
Heroin
Intoxication: Sedation, euphoria, pinpoint pupils, unconscious , shallow respiration,
Can be snorted, injected, smoked
Withdrawal: anxiety, craving, restlessness, yawning, nausea, sweating,aches and muscle pain, diarrhoea
Methadone and buprenorphine
Methadone- synthetic opioid
Dual diagnosis
30-50% of patients with psychotic illness have substance use disorder
Over 30% of those with substance disorder have psychiatric disorder
Cannabis
Hashisi
Euphoria, relaxation, showed reaction time, distorted sensory perception, impaired balance and coordination, increased heart rate and appetite, impaired learning, memory
Risk of cough, frequent respiratory infection, possible mental health decline, addiction
Hallucinogen
LSD, mescaline, psilocybin, Magic mushroom
Altered state of perception and feeling, hallucination, nausea, increased autonomic activity
For LSD-flashbacks, hallucinogen, persisting perception disorder
Opioid
Heroin
Opium
Euphoria, sedation, slow breathing
Risk of constipation, endocarditis, hepatitis, HiV, addiction
Dissociative drugs
Ketamine/ PCP and analogues
Feeling of being separate from ones body, impaired motor function, analgesia
Risk of anxiety, tremors, numbness, memory loss, nausea
Stimulant
Cocaine, amphetamine, methamphetamine
Stimulant physically (energy, tremor) and mentally (alert, exhilarated), psychologically anxious, irritable
Risk of weight loss, insomnia, cardiac or cardiovascular complication, stroke, seizure, addiction
Club drug
MDMA
Mild hallucination , increased tactile sensitivity, lowered inhibition, anxiety sweating, muscle cramping, tech clenching
Risk of sleep disturbance, depression, impaired memory, hyperthermia
Nicotine
Increased blood pressure and heart rate
Risk of chronic lung disease, cardiovascular disease, stroke, cancer of mouth, adverse pregnancy outcome
Alcohol
Euphoria , mild stimulation, relaxation, lower inhibition,
Drowsiness, impaired memory, sexual dysfunction, unconscious
Risk of injury, violence, fetal damage, depression , neurologic deficits, liver and heart disease, addiction
Alcohol misuse definition
Hazardous: drink over recommended limit
Harmful: experienced health problem
Dependent: withdrawal symptoms
