  • Are they short of breath? — Is this normal?
  • How far can they mobilise? Do they use an aid? What eases their shortness of breath?
  • Do they normally have a cough? — Is it productive? What colour is it? How much do they bring up?
  • Do they have a wheeze? - What makes this better/ worse?
  • Do they fatigue easily or become dizzy?
  • Do they have any pain?

  • What and when – what – are they short of breath? Do they normally have a cough?
    • when - are patients symptoms acute, subacute or chronic – is this the first onset or did symptoms start a few years ago? When different symptoms started, did they come on together?
    • H – how did it start?
    • I- Interventions? What treatment have you had to date? Any Investigations? Recent CXR? Blood tests?
    • P- Pattern since onset? Worse staying same or getting better? Pain? Plan for any further treatment?
