Purpose:
A
- Provide medication into subcu tissue for absorption
2. To allow slower absorption of a medication compared w/ either the IM or intravenous route

Equipment
A
Only differences:
- Adhesive bandage or band aid
- 2 to 3 mL syringe with 1/2 to 7/8 in needle (25, 26, 27 gauge needle) or insulin or tuberculin syringe

- Agency protocol on specific sites of subcu injection
- Condition of client’s skin (RHSSTALEEhEIT)
- Medication allergies
- Specific drug action, side effects, and adverse reactions
- Client’s knowledge and learning needs about medication
- Ability and willingness of client to cooperate during the injection
- Previous inject sites used

Special Consideration
A
- Check agency procedure manual before heparin or insulin administration. Some agencies recommend that aspiration after needle insertion should NOT be performed with heparin administration. Many agencies require that heparin and insulin be double-checked by another nurse during preparation for accuracy of drug, amount, and client.
- Pedia: children younger than 1 yr, the vastus lateralis is preferred (.5 mL for small children)

- HH and ICP
- Prepare the medication from ampule or vial adhering to 12 rights
- Provide for client privacy
- Prepare the client

- Assist to a position which the arm, leg, or abdomen can be relaxed depending on site (relaxed and minimizes discomfort
+ Obtain assistance holding uncooperative client (prevent injury due to sudden movement after needle insertion)

- Explain the procedure, purpose of the medication and how will it help future care using the language that the client understands. Include info abt effects of medication. (Facilitate acceptance and compliance, promotes cooperation and reduces anxiety)
