Ampule Administration Flashcards
1
Q
Purpose
A
Obtain medication from ampule using aseptic technique for administration by a parenteral route
2
Q
Equipments (13)
A
Remember:
- Appropriate size syringe and filter needle depending on type of injection and viscosity of solution
- 2x2 sterile gauze pad
- Ampule file
3
Q
Assessment
A
Appearance of solution (C, A, C)
Expiration
Allergies and sensitivity
4
Q
General Considerations
A
- Maintain sterility while preparing using asepsis technique
- Never use drug that is cloudy, discolored, or contains a precipitate unless indicated by manufacturers
5
Q
Preparation:
- Assemble Equipment
- Check Medication Card or Doctor’s Order
A
- +Assemble Equipment
Rationale: Facilitates organization, save time and effort and promotes efficiency
Remember:
- File (if ampule is not scored) and small gauze square or plastic ampule opener
- Filter needle for drawing medication from the ampule
+ Assess the
Rationale: Promotes safe drug administration and prevents allergic reactions and adverse event
- Appearance
- Expiration
- Allergy - + Check the label on the ampule against the MAR or doctor’s order to make sure it is the correct medication
+ Follow 3 Checks
Rationale: Promotes safe drug administration and correct drug administration
6
Q
Procedure
1. HH and ICP
2. Check label of ampule with the MAR adhering to the 12 rights of drug administration. Use barcode scanning if available
Note: Perform dosage Calculation if different dosage required.
A
- Prevent the spread of microbes
- Promotes safe drug administration
- determines correct amount will be withdrawn
7
Q
- Prepare medication for drug withdrawal
- flick upper stem with fingernail
- tap upper neck with fingertips
- rotate with wrist
A
Displaces solution from top of ampule to bottom and ensures that all of the solution is withdrawn and there will be no wastes
8
Q
- Place alcohol swab or sterile gauze pad around neck of ampule with fingers of dominant hand and nondominant hand on lower part
- With quick snapping motion of wrist break top of ampule away from you
- Place top of ampule on paper towel or discard immediately on the sharp’s container
A
- Promotes easy opening of ampule and stabilizes ampule and protection from finger cuts
- Opens ampule without injury
- Proper waste disposal and prevent’s injury
9
Q
- Withdraw the Medication:
- Place ampule on flat surface and remove cap from filter needle and attach filter needle to syringe - Press plunger all down and not aspirate air
- Insert needle into center of ampule without the hub touching the edges of the ampule
- Withdraw appropriate amount of solution and remove needle from ampule
A
- Stabilize ampule and easy attachment of filter needle
- Prevents accidental displacement and waste of solution
- Maintains sterility
- Ensures proper and correct dosage of medication syringe
10
Q
- Place ampule on paper towel and discard.
Note: single dose ampule, tilt it and withdraw all solution and dispose. - Bubbles
- vertically
- pull and flick
- push - Recheck amount and compare
A
- Prevents injury
- Removes air bubbles
- ensures that correct amount is prepared
11
Q
- Compare drug label
- Change needle if drug is known to cause tissue irritation and replace cap
- Label syringe w/ name and dosage
- Place syringe, MAR, and additional AS on med tray
A
- Provides additional identification check
- Prevents tissue irritation since drug may have attached to the syringe or any glass shards
16 and 17. Organizes materials and safe drug administration
12
Q
- Replace filter needle with regular needle and tighten the cap at the hub of the needls and push the solution into the needle to the prescribed amount
- Restore/discard all equipment
- HH
- Document
- Name of med
- Date and time withdrawn
- dosage drawn
- Med is controlled substance follow agency policy and Controlled substance record book
A
- to avoid tissue irritation and maintain sterility
19 and 20. Reduces transfer of microbes - Proper documentation, reporting, and recording.
