Stroke Rehab Flashcards
CVA – cerebrovascular accident
rapid onset of function disturbance lasting >24 hrs. no apparent cause other than vascular origin. “brain attack”
TIA – transient ischemic attack
<24 hrs. mini-stroke
CVA correlation with
cigarrettes, HTN, and CT
post CVA death
1 mo. Effect of brain lesion or complications of immobility. Within 6 mo. Heart disease.
Number one modifiable risk factor of Stroke.
HTN
Goal of stroke rehab
return maximum recovery of physical, cognitive, and psychological function. Optimizing independence, life quality, and patient dignity. Multidisciplinary.
Physiatrist
provides primary medical care, while coordinating interdisciplinary team and medical specialists.
what is the most common stroke impairment?
hemiparesis, relatively predictable
Twitchell – progression post stroke
Immediate – decreased voluntary movements and stretch reflexes. 48 hrs – increased stretch reflexes (MSR). 1-38 days – spasticity and clonus. 6-33 days – Increased voluntary movement, decreased spasticity. Motor control returned proximally before distally. Lower extremeties recover faster than upper.
complete paralysis of arm at onset correlates with…
poor prognosis in useful hand function.
some movement of hand by 4th week correlates with…
good chance of making a full recovery.
severe proximal spasticity correlates with…
poor prognosis.
Most common ischemic CVA
Thrombotic – large artery during sleep.
shoulder pain
75% of pts with hemiplegia – developed durring spasticity phase. Ddx – CRPS, subluxation
Transcortical sensory vs Wernicke aphasia
wernicke has no repetition
