What are common causes of large vessel ischemic stroke?
Emboli (30-40%) mainly from heart or carotid arteries. Also, atherosclerosis or aneurysm
What is the therapy for acute stroke?
IV tpa within 3 hours of onset of symptoms and intra-arterial therapy with stent retrievers within 6 hour window (Screen with noninvasive imaging)
What determines the success of TPA?
the length of the clot
What is the most sensitive way to look for an acute stroke?
Diffusion weighted imaging- should see bright signal
What does a CT perfusion look at?
How contrast flows through a brain tissue- Cerebral blood volume and mean transit time. If these do not match, it probably means that there is still some viable tissue
