What are the contraindications to tPA?

A
  • Age > 80
  • NIHSS > 25
  • Oral anticoagulation (irrespective of coag studies)
  • INR > 1.7
  • Platelets < 100,000
  • Hx diabetes with previous stoke
  • Head trauma in last 3 months
  • Glucose < 50 (can increase, then give tPA)
  • Glucose > 300 (can reduce, then give tPA)
  • BP > 185/110 (can reduce, then give tPA)

A

Reduce BP to < 185/110 using labetalol or nicardipine

Then, give tPA unless there is another indication.
