qSOFA criteria
qSOFA = Quick sequential organ failure assessment
Two or more of the following:
- RR > 22
- SBP < 100 mm Hg
- Altered mental status
A score of 2 or greater suggests sepsis. If this is the case, a full SOFA score should be performed, and the patient will likely need acute care and ICU admission.
SIRS criteria
Systemic inflammatory response system
Two or more of the following:
- Temp > 38 oC OR < 36 oC
- Heart rate > 90 bpm
- RR > 20 breaths/min OR pCO2 < 32 mmHg
- WBC > 12,000 OR < 4,000 OR >10% bands
SIRS vs qSOFA score
SIRS is too sensitive and not specific enough
qSOFA is still quite sensitive, but is more specific for sepsis.
Elevation of the RR above __ carries a poor prognosis in multiple clinical contexts (pneumonia, CHF, etc)
Elevation of the RR above 22 carries a poor prognosis in multiple clinical contexts (pneumonia, CHF, etc)
When pulse pressure is suggestive of poor stroke volume
A pulse pressure below 25 mmHg suggests poor stroke volume, which may indicate CHF or shock.
1 poor prognostic for all-comer hospital mortality
RR > 22
2 poor prognostic for all-comer hospital mortality
Altered level of consciousness
Mortality risk of a qSOFA score >=2
10%
