Stress cardiomyopathy is also known as
takotsubo cardiomyopathy
ischemic chest pain with EKG changes in the absence of obstructive coronary artery disease
what causes stress cardiomyopathy?
Caused by excessive catecholamine stimulation, causing myocardial stunning and microcirculatory dysfunction and multivessel coronary artery spasm.
seen in post menopausal women after an intense, stressful, emotional or physical situation
Treatment of stress cardiomyopathy
IVF, treatment of complications and clinical monitoring
some physicians recommend BB indefinitely
prognosis of stress cardiomyopathy
most recover ventricular function within 8-12 weeks
EKG TTE findings of stress cardiomyopathy:
ST seg elevation in anterior leads (50% of pts)
Deep T wave inversions, Q waves, prolonged QT in some pts
mildly elevated troponin
TTE with LV apical ballooning with akineses or dyskinesis and decreased EF and basal hyperkinesis
Coronary angiography with LV apical ballooning but normal coronary anatomy
