Stomach and duodenal disorders Flashcards
Consituents of GOR
Bile, bile salts, lysolecithin, acid, pepsin, pancreatic enzymes
Described location of heartburn
Just below sternum or between shoulder blades
GORD describes which 3 conditions?
Heartburn, oesophagitis, acid reflux (with or without pain)
Detection of acid reflux
Mouth or back of throat
Consequences of GORD
mucosal damage, oesophagitis, barretts oesophagus, adenocarcinoma
Define Barretts oesophagus
Barrett’s Oesophagus is a condition where the normal lining in the gullet becomes very thin and the opening enlarges
4 causes of reflux
Weakness of sphincter
High pressure within stomach
High pressure from abdomen
Hiatus hernia
Drugs used for reflux relief
H2RA - ranitidine, cimetidine
PPI - omeprazole, lanzoprazole
ProKinetics - metoclopramide, domperidone
Alginates - rafts and viscosity, gaviscon
Describe a nissen-fundoplication
fundus of stomach wrapped around distal oesophagus
What characterises dyspepsia
Abdominal discomfort, bloating and early satiety
Causes of dyspepsia (not serious)
GORD, poor eating habits, smoking, NSAIDS,
Serious causes of dyspepsia
Gallbladder inflammation, gastritis, gastropariesis, PUD, h.pylori, cancer, lactose intolerance, IBS
Factors usually preventing reflux
Alkaline saliva, peristaltic squeeze, intra-abdominal squeeze, diaphragmatic squeeze, LOES, intragastric squeeze, mucosal choke
2 types of gastritis
Chronic atrophic and acute erosive
Causes of chronic atrophic gastritis
Hpylori infection, ongoing acute gastritis
