1: - PPE + scene safe - ENAMES - General Impression - AVPU / A&O x _ - Chief Complaint - A, B, C’s + skins signs/Lung sounds + possibly give O2 - Transport decision —- History: - OPQRST/PASTE/AEIOUTIPS + SAMPLE —- 2: - Affected body part - Pertinent negatives - Vitals (BP, HR, RR, SpO2, Pupils, lungs, temp) - Field impression and interventions - Transport and code (treat for shock) - Reassess (every 5 or 15 min) - Turnover - demographics (who? What? Where?) - chief complaint/vitals - pertinent history - intervention and response
