Signs/Symptoms of Herpes Simplex Virus
Burning or painful erythema, papules, multiple vesicular or ulcerative lesions
HSV 1 - oral
HSV 2 - genital
Diagnosis of Herpes Simplex Virus
PCR of genital specimens (Expensive test of choice). Serologic assays. Direct flourescent antigent. Culture.
Treatment of Herpes Simplex Virus
Antiviral Drugs
1st clinical episode, treat for 7-10 days: Acyclovir 400 mg tid. Acyclovir 200 5x/day. Famciclovir 250 tid. Valacyclovir 1g bid.
Suppresive therapy: for 6+ recurrences/year: Acycovir 400-800 BID. Famciclovir 250-500 BID. Valcyclovir 1g QD.
Pregnancy and Herpes Simplex Virus
Dont have sex with sero-positive partners in 3rd trimester
Herpes present at labor –> C Section!
Primary Syphilis
Signs: painless single papule. Painless clean ulcer with smooth base and firm raised borders. Multiple chancre in HIV pts. Painless lymphadenopathy.
Labs: Darkfield exam
Seconadry Syphilis
Signs: Macular/maculaopapular/papular/pustular syphilides (palms and soles) Condyloma lata. Mucus patches. Constitutional symptoms (fever, malaise, weight loss, pharinitis, arthralgia). CNS involvement.
Condyloma acuminatum TX
podophyllin resin. purified podophyllotoxin. 5-fluorouracil. surguery. cryotherapy. electrocoagulation. laser therapy. intralesional inferferon-alpha. imiquimod. 3% cidofovir.
Tertiarty Syphilis
