Oxacillin/nafcillin, or cefazolin (first-generation cephalosporin)
Staph Sensitive (MSSA): Oral
Dicloxacillin, or cephalexin (first-generation cephalosporin)
Resistant Staph (MRSA): Severe Infection
Vancomycin, Linezolid (s/e: thrombocytopenia), Daptomycin (myopathy, increased CPK), Ceftaroline, tigecycline, or telavancin.
Resistant Staph (MRSA): Minor Infection
Trimethoprim/Sulfamethoxazole, clindamycin, doxycycline
Penicillin Allergy (staph): Rash, Anaphylaxis, Severe, Minor infection
Rash: Cephalosporins; Anaphylaxis: Macrolides (azithromycin, clarithromycin), or clindamycin; Severe infection: Vancomycin, Linezolid, Daptomycin, Telavancin;
