Q

Point of initial tooth contact when
the condyles are guided along the
posterior slope of the articular
eminence into their most superior
position on jaw closure
Synonyms: centric relation
occlusion, retruded contact
position

A

Centric Relation (CR) = Retruded

with the most complete
interdigitation of
opposing teeth
independent of

Synonyms: maximum

(MIP), maximum
intercuspation,
intercuspal position

Q

We believe that the
_____ decreases adverse
loading forces on the
TMJ and muscles of
mastication which can
cause trauma &
inflammation resulting in
pain.

7
Q

Effective in
pediatric patients
Less effective in
adults
Protects teeth from
trauma
Does not decrease
bruxism; may
increase bruxism in
adults
May be helpful for
short-term use
Inexpensive

A

8
Q

Effective in pediatric
patients and used for
MIXED DENTITION
because it is SOFT and
will not block the
eruption of the
permanent teeth
Useful in treating:
Myalgia, Arthralgia,
and
clenching/bruxism
Less effective in adults
Protects teeth from
trauma
Does not decrease
bruxism; may increase
bruxism in adults
May be helpful for
short-term use
Inexpensive

Q

◦ Treats anterior disc displacement
without reduction & intermittent
non-reduction
◦ Has indexing or indentations
holding jaw anteriorly to reduce
inflammation on the posterior
attachment reducing acute pain

A

11
Q
  1. Acrylic (hard) splint with flat-
    plane occlusion; clasps add
    retention OR acrylic with flex
    liner (hard/soft splint)
  2. Must cover all of the maxillary or
    mandibular teeth
  3. Hard appliance reduces
    symptoms more quickly &
    effectively than soft splint
  4. Good for bruxism/clenching
  5. Reduces symptoms in 70-90% of
    TMD’s

Q

Indications
Anterior disc displacement without reduction with
significant pain in TMJ
Anterior disc displacement with intermittent non-
reduction (catching)
Primarily indicated for ACUTE TMJ pain associated with
disc displacement with reduction

16
Q

◦ MAY Affect TMJ Disc Displacements in 3 Ways:
1. Alters adverse loading in the TMJ (unloads the posterior
attachment)
2. Alters the structural condyle-disc relationship
3. Reduce associated muscle splinting

A

Anterior Repositioning Splint

17
Q

◦ Can be fabricated on either arch
◦ Take occlusal wax registration with
mandible moved anteriorly (not greater than
3mm) which hopefully eliminates click- listen
with a stethoscope
◦ Posterior occlusal indentations & anterior
guidance incline passively guide mandible
anteriorly
◦ Used in selected cases

A

Anterior Repositioning Splint

18
Q

Not always successful in
recapturing disc or eliminating
TMJ noises
Night time wear is indicated if
jaw locking primarily occurs
on awakening
Day time wear is indicated if
locking with pain primarily
occurs in daytime
Replace with stabilization
splint in 6-12 weeks once joint
pain/dysfunction is controlled

A

Anterior repositioning splint

19
Q

“Anterior jig”
Covers maxillary incisors only
Can be easily swallowed
Posterior teeth could possibly super- erupt
Causes posterior teeth to disclude
For short term treatment ONLY
NOT RECOMMENDED

A

Nociceptive trigemino- inhibitor
(NTI) or Anterior Bite Appliance

20
Q

What conditions require 1 month recalls for 6 months after splint delivery?

A

Arthritis and osteoarthritis
