Multiple symmetric hoing involvement
Significant joint involvement
Morning joint stiffness longer than 1 hour
Symmetric spindle-shaped swelling of proximal interphalangeal joints and volar sublaxation of metacarpophalengeal joints
Systemic manifestations
Rhematoid arthritis
Usually one or two joints involved
Joint pain without inflammation
Morning stiffness of less than 15 mins
Heberden’s nodes of distal interphalageal joints and Bouchard’s nodes of PIP joints
No systemic involvement
Osteoarthritis
- ONe of more of the following:
Muscle pain report
Muscle pain opening
Muscle pain on excursion - Muscle pain on palpation that does not refer to different locations
Myalgia
Regional dull, aching muscle pain aggravated by mand function
Trigger points are present that increase the pain complaint and refer the pain
Myofascial pain
- ONe of more of the following:
Joint pain report
Joint pain on opening
Joint pain on excursion
2.Joint pain on palpation - Abscence of crepitus noises
- Abscence of degenerative changes on Pano or other TMJ imaging
Arthralgia
- ONe of more of the following:
Joint pain report
Joint pain on opening
Joint pain on excursion
2.Joint pain on palpation - Crepitus in joint during any movement may be present
4.Degenerative changes are evident on PANo or other imaging of TMJ
Osteoarthritis
- Neither joint pain report or joint pain on palpation
- Crepitus in joint during any movement
OSteoarthrosis
- Clicking on opening and or closing
- Corrected deviation of mand to affected side on mouth opening/closing
- MRI would reveal an anteriorly displaced disc which seats on condyles with mouth opening
Anterior disc displacement with reduction
- Limited mouth opening (assisted) of less than 35mm
- Deflection of mand to affected side on mouth opening
- MRI would reveal an anteriorly displaced disc which does not seat on condyles with mouth opening
Anterior disc displacement without reduction
