Causes of isolated splenomegaly
A
Lymphoproliferative disorder (but usually includes also lymphadenopathy and hepatomegaly) (Massive in CML/myelofibrosis, less so in CLL, lymphoma, cirrhosis, EBV, CMV and SBE)
Chronic malaria
Kala-azar (visceral leishmaniasis)
Schistosomiasis

A
Feltys syndrome
Chronic haemolytic anaemia
Infective endocarditis
Left sided portal hypertension (portal or splenic vein thrombosis (consider pancreatic mass)
