Splenomegaly Flashcards
Structure
A reticuloendothelial organ
spleen
Embryonuc origin
At 5th week of gestation in dorsal mesogastrium
Migrated to LUQ,
attatches to stomach via gastrolineal ligament and to kidney via lineorenal ligament
Accessory spleen develops if
Hillocks fail to unify
Physiological function of spleen
Sequestration of defective rbc
Synthesis of antibody in white pulp
Remove antibody coated bacteria and antibody coated blood cell from circulat
Spleen composed of
Red pulp and white pulp
Which are malphigus term for red blood filled sinuses and reticuloendothelial cell lined cords and white lymphoid follicles arrayed within red matriX
Spleen
Is in portal circulation
Blood flow in spleen
150 ml/min through splenic artery finally ramifies into central arterioles
Fate of blood entering splenic circulation
Majority of blood from central arterioles flow to macrophage filled sinus and cords and capillaries-veins
Blood entering sinuses reenter circulation via splenic venules
Entering cords…undergo a inspection..to enter the circulation blood in the cords should squeeze through slits in the cord lining to enter sinuses that lead to venuels
What happend to old rbc in cords?
Old and damaged erythrocytes are less deformable so they are retained in the cords , where they are destroyed and compartments are recycled
Pitting
Process of passing through slits
Pitting can remove red cell inclusion bodies such as parasites, nuclear residua, howell jollybodies, denatured hemoglobin(heinz bodies
Y pitting and culling should go simultaneouly
It should occur without significant delay bcoz blood transit time in spleen is slightly lower than in other organs
Spleen assist in
Help host in adapting to hostile environment
Clearamce of bacteria and particulated from blood
Generation of immune response to certain pathogens
Generation of cellular components of blood under circumstances in which bone marrow is unable to meet the needs
Extramedullary hematopoesis
The spleen help in production of components of blood when bone marrow is unableto meet the needs
Spleen
some animals can store rbc in spleen
Not in humans
In humans one third of platelets and marginated nuetrophils are present
