spinal cord injury Flashcards
complete SCI
no function below injury
incomplete SCI
partial sensation and function
SCI below T1
paraplegia (lower ext)
injury to cervical region
quadriplegia (all 4 ext)
above L1 or L2
hypertonia (spastic muscle tone)
below L1 or L2
hypotonia (flaccid muscle tone)
hyperextension MOI
sudden acceleration
rear end collision and falls when chin is pushed up
hyperextension results in
posterior compression and anterior ligament may stretch or tear
hyperflexion MOI
sudden deceleration
head on collision or driving accidents
hyperflexion results in
ruptured or torn posterior ligaments
compression MOI
extreme force to the spine
driving accident, fall on feet or buttocks
rotation MOI
lateral flexion or twisting of head and neck
T bone, MVA, falls
rotation results in
tear or rupture of posterior ligaments, dislocation of facet joints, fractures at articular processes
secondary injury
biological processes that occur after initial traumatic event leading to further damage of spinal cord
how to decrease secondary injury (drug)
immobilize and stabilize
airway mgmt
physiological support
methylprednisolone (corticosteroid)
diagnosis of SCI
x ray
CT scan
MRI
angiograph
SEPs (somatosensory-evoked potentials)
physical assessments
assessment of motor, sensory, and reflex function
spinal shock
occurs 30-60 mins after injury
absence of reflexes, flaccidity, loss of sensation
treatment is symptomatic
neurogenic shock
above T6
loss of SNS= hypotension, bradycardia, warm flushed skin
cardiac dysfunction
neurogenic shock treatment
fluids and vasopressors, atropine
resp complications of spinal injury
C1-C2 require mechanical ventilation *pneumonia risk
C3-C5 diaphragm paralysis
below C6 intercostal and abd muscle impairment
autonomic dysreflexia
exaggerated sympathetic response from above T6
s/s hypertension, sweating, goosebumps, headache, blurred vision
autonomic dysreflexia treatment
SIT PT UP
give antihypertensive
treat underlying cause (ex: distended bladder or bowel)
perfusion complication
ortho hypotension
neuropathic pain therapy
NSAIDS
opioids
antidepressants
anticonvulsants
cannabis
spastic bladder
above T12
incomplete bladder emptying
flaccid bladder
below T12
incontinence
