special care Flashcards
what is chemotherapy induced oral mucositis
painful inflammation and ulceration of the mucous membranes lining the mouth, caused by the toxic effects of chemotherapy drugs. It is one of the most common and debilitating complications of cancer treatment
treatment of mucositis
- avoid spicy, acidic and hard foods
- CHX if too painful to brush
- opioid analgesics often required
- cryotherapy, laser therapy
impacts of chemotherapy relevant to dentistry
dry mouth during treatment
mucositis
immunosuppresion
coagulation defects
impacts of radiotherapy relevant to dentistry
radiation caries
mucositis
long term dry mouth
trismus (fibrosis of MOM)
ORN risk
loss of taste
2 aims of treating patient about to begin cancer treatment
remove or treat current infection
institute prevention
4 patient groups at increased risk of infective endocarditis
previous IE
congenital heart defects
valve replacements
hypertrophic cardiomyopathy
describe IE to a patient and why is it relevant to dental treatment
IE means an infection of the inner lining of your heart and valves. Caused by bacteria gaining access to blood stream and settling on the inner heart tissues. Without treatment can lead to serious complications such as heart failure and even death.
some dental treatments have been implicated in allowing bacteria access to the bloodstream
bridging
technique for helping carers with oral care of residents
Show and explain to patient what toothbrush is
give them spare toothbrush
mimic brushing your own teeth
they will hopefully mirror
chaining technique for assisted oral care
bring pt hand with toothbrush to mouth while describing toothbrushing
hand over hand assisted oral care
place hand over patients and gently brush together
order of trying assisted oral care from least input to most input
- bridging
- chaining
- hand over hand
- distraction
