What bacteria cause purulent cellulitis?
Staph aureus/MRSA
What are rash red flags?
travel, fever, new meds, immunocompromised/old/young
How do you treat purulent cellulitis?
SMX/TMP, vancomycin IV, doxycycline, clindamycin
What is the etiology in non-purulent cellulitis?
Streptococcus pyogenes
What is the tx for non-purulent cellulitis?
dicloxicillin, amoxicillin, nafcillin IV, clindamycin, macrolides, cephalexin, cefazolin IV
What is the etiology of erysipelas and what are s/sx’s?
Strep pyogenes. intense erythema on low extremities or face, induration, sharp border, fever, chills, jt/muscle pain, HA, nausea
What is the etiology of necrotizing fasciitis?
type I = polymicrobial, includes Vibrio sp, includes Fournier’s gangrene, type II = Group A strep (S pyogenes), Type III = Clostridium sp.
What are s/sx’s of necrotizing fasciitis?
intense pain/tenderness, prev trauma, fever, malaise, myalgias, edema, crepitus/cutaneous emphysema, pain progresses to anesthesis, hx DM, erythema, purple discoloration, black spot
Tx of necrotizing fasciitis
surgical debridement, pen G + aminoglycoside (Zosyn) + clindamycin + vancomycin
What is pemphigous vulgaris?
autoimmune blistering dz, IgG mediated, oral & cutaneous lesions
