A wound is defined as:
a break or disruption in the normal integrity of the skin and tissues
Closed wound
edges well approximated - no gapping
Open wound
Stage 3 or 4 pressure injury
could also be a wound that is intentionally left open for healing
Clean wound
typically a surgical wound - use sterile technique
Dirty wound
decubitus or pressure injury - use clean technique
Incision wound
edges well approximated
Contusion wound
bruise
Abrasion wound
rubbing or scraping off of the skin’s outer layer
Puncture wound
wound caused by a sharp, pointed object piercing the skin
Laceration
tear in the skin
Partial Thickness
Dermis and epidermis
Full thickness
Down to subcutaneous/muscle/bone/tendon
Acute wound
Usually heals up in 21 days
Chronic wound
can take up to months or years to heal
Normal healing of skin requires:
Circulation
Clean environment
Nutrition
Primary Intention
closed with suture material
Secondary Intention
will heal from wound bed up
Extensive tissue loss
Repair time longer
Scarring is greater
Susceptibility to infection is greater

Maturation or Remodeling Phase
Stage 1 of wound healing - hemostasis
blood clotting - usually happens within hours
Stage 2 of wound healing - Inflammatory Phase
Red, sore, swollen - usually the first 4 days
Stage 3 of wound healing - Proliferation Phase
macrophages move in to clean up wound
