a baby is born and they are not breathing or are purplish white. what do you do?
check HR, if absent, no need to resuscitate. these conditions suggest the baby died before being born.
a baby is born and is not crying even while being stimulated. what do you do?
a baby who isnt crying, isnt breathing. start ventilations
clearing airways of newborns
1) hold up jaw to extend head
2) check airway, if not secretions are blocking airway, do not suction
a) if there is meconium in the amniotic fluid and baby is not breathing, suction secretions from nose and mouth
how do you stimulate a newborn who is not crying/breathing?
turn baby and rub back a few times
what are common reasons for no chest rise during ventilations?
1) hyperextended neck (blocked airway)
2) air leak between mask and face
3) mask too large
what do you do if you dont see chest rise in a baby during ventilations?
1) position baby’s head till its slightly extended
2) reapply mask to make a better seal
3) if still no chest rise, remove mask and suction secretions from mouth and nose
4) try ventilating again
5) if still nothing, squeeze bag harder
when do you check HR during ventilations?
after at least 30 seconds of PPV (check pulse at umbilical cord, if cant be felt use stethoscope on chest)
when do you stop efforts to resuscitate?
if no heart beat @ birth and no heart beat after 10 minutes of resuscitation, stop resuscitation.
a baby is born at term and is not breathing or crying and does not have a good tine, what do you do?
warm, dry, stimulate, position airway, and suction if needed
after a baby is warmed, dried, stimulated, airway is positioned, and its been suctioned but still has apnea or HR is <100bpm. what next?
PPV, pulse oximetry

when do we start rescue breaths on a newborn?
- PPV for baby with no respiratory effort/HR below 100 bpm
- CPAP to support spontaneous breathing that is labored
how do we count breaths?
breath, two, three, breath…
when do we start CPR on a newborn?
HR <60bpm
what is the ratio of breaths to compressions?
3:1 3 compressions 1 breath
compression techniques:
1) two finger
2) thumbs encircling
* 1/3 AP diameter deep
MR SOPA:
M -mask adjustment - consider 2-hand technique
R - reposition airway: head neutral or slightly extended
* - try PPV and reassess chest movement
S - suction mouth and nose - bulb syringe or suction catheter
O - open mouth and lift jaw forward
* - try PPV and reassess chest movement
P - pressure increase - increase pressure in 5-10cmH2O increments (max 40)
* - try PPV and reassess chest movement
A - alternative airway - place endotracheal tube or laryngeal mask
* - try PPV and assess chest movement and breath sounds
