What to look for on perianal exam?

Prehn’s Sign for testicular pain
Elevate scrotum while supporting testicles
If pain is decreased = +Prehn’s sign and suggestive of Epididymitis
If pain is the same or increased it is a – Prehn’s sign and suggestive of testicular torsion
Cremasteric reflex
Lightly stroke the upper inner thigh, the scrotum should rise on the same side as the stimulation.
Loss of the reflex is suggestive of testicular torsion in a patient who presents with testicular pain.
