What is Sjogren’s?
- Autoimmune inflammatory disorder
- Diminished lacrimal and salivary gland secretion
Two forms of Sjogrens
Primary - not associated with other diseases
Secondary- associated with underlying rheumatic disease
Male or female?
80% female
Common symptoms of Sjogrens
- Myalgia
- Arthralgia
- Dry mouth
- Fatigue
- Raynaud’s
- Enlarged parotids
- Dry eyes
MADFRED
Presentation of Sjogrens
Sicca symptoms:
* Xerophthalmia (dry eyes)
* Xerostomia (dry mouth)
* Fatigue
Investigation for Sjogrens
- Schirmers test - filter paper in eye, measures tear volume
- Salivary gladn biopsy?
Antibodies in Sjogrens
- Anti-Ro and Anti-La
- RF and anti-dsDNA may be seen
Treatment for Sjogrens
- Symptomatic
- Avoid dry/smoky atmospheres
- Dry eyes - artificial tears
- Dry moith - artifical saliva, sugar free gum/pastilles
- Skin emoillients
- Vaginal lubricants
- Immunosupression/steroids rarely needed
Common diseases associated with Sjogrens
- SLE
- Systemic sclerosis
- Rheumatoid arthiritis
Other autoimmune conditions associated with Sjogrens
- Coeliac
- Primary biliary cirrhosis
- Autoimmune thyroid disease
Serious risk associated with Sjogrens
- 5-10% lifetime risk B cell lymphoma
Pregnancy and Sjogrens
If positive for anti Ro, risk of:
* Fetal loss
* Complete heart block fetus
* Neonatal lupus syndrome in newborn
Biopsy for Sjogrens
Focal lymphocytic infiltration of exocrine gland
