What are some triggers of SIRS?

A

Mechanical tissue trauma, abcesses, ischemic or necrotic tissue, microbial invasion, perfusion problems

What is the definition of SIRS?

A

Systemic inflammatory response syndrome: cytokines cause a generalized inflammatory response and the whole body thinks it’s inflamed

What is the process of SIRS?

A

Cytokines go everywhere so wbcs go everywhere, histamine dilates everywhere, all vessels become permeable, fluid and albumin leaks out all over the body, liver keeps making clots, and the body goes into a hyper metabolic state

What is relative hypovolemia?

A

A decreased circulating volume; the fluid is not in the vessels and there is a decreased cardiac output

How do ketones form in the body during SIRS?

A

The liver is releasing sugar as the body demands it, but it eventually runs out of glycogen so it converts amino acids; now the protein and albumin is low and the byproduct of burning amino acids is ketones.

What are the clinical signs of SIRS?

A

Must have two or more to be considered sirs: high or low temp, HR greater than 90, RR greater than 20, CO2 less than 32, wbc greater than 12 or less than 4 and immature bands greater than 10%

What is uncomplicated sepsis?

A

It has all signs and symptoms of SIRS, inadequate tissue perfusion, and a documented infection

A

Complicated sepsis with organ dysfunction, tissue hypo perfusion with a lactate level greater than 4, an hypotension
