What silver diamine treatment is
It is a simple treatment option which may be suitable for patients who cannot accept conventional dental treatment
The solution is applied onto the tooth cavity and can arrest the decay.
Silver diamine is antibiotic liquid. It can treat tooth sensitivity and decay. Almost effective when applied twice yearly.
It is licenced to be used in UK for dental sensitivity not decay.
It is clear, odorless liquid for desensitisation of non-carious tooth lesions and MIH
Advantages and disadvantages of silver diamine
Advantages: may relieve sensitivity
May prevent abscesses by slowing down or stopping tooth decay
Can buy time for pt too young /with additional needs who cannot manage routine treatment
Can avoid fillings/xla by stopping the decay
Disadvantages:
Painted area will stain black permanently (healthy parts will not stain)
If accidentally applied to the skin/gums- will stain it
It will stain clothes as well
Metallic taste may be noticed during the application but goes away rapidly
Procedure of SDF
- Drying the affected area
- Placing a small amount of SDF on the affected area
- Allowing SDF to dry for 1 min
- Rinse with water
Theory behind it
Silver is antimicrobial
Fluoride concentration 38% (it incorporates into the dentine, hardens it)
44,8000 ppm Fluoride
25,780 ppm silver
*Silver works by inhibiting the reproduction of the microbe by:
Silver ions breaking through the cell wall, silver ions disrupting the cell and attaching to the DNA of the microbe to stop cell replication
It will promote tooth desensitisation by dentinal tubules blockage, bacterial death, remineralisation of demineralized tooth and inhibition of dentinal collagen degradation.
Dosage and use
Only one capsule to be used in a single session
Maximum number of teeth that can be treated in one session is 4
A review appointment should be arranged in 2-4 weeks
Reapplication of SDF may be required after 6 months
