2
Q
Components of shoulder inspection
A
- Inspect anterior and posterior shoulder with patient standing for
- Deformities
- Asymmetry
- Swelling
- Redness
- Nodules
- Muscle atrophy
- Etc?
3
Q
Components of shoulder palpation
A
- Anterior and posterior
- “I am palpating for tenderness, bogginess, and bone enlargement, etc.”
- Begin at SC joint
- Along clavicle to AC joint
- Include subacromial bursa
- Scapula
- Deltoid
- Biceps tendon
4
Q
Active ROM
A
- Flexion
- Extension
- Adduction
- Abduction
- Internal rotation
- External rotation
- External rotation with abduction (hands behind the neck)
- Internal rotation with adduction (hands behind the back)
5
Q
Passive ROM
A
- Flexion
- Extension
- Internal rotation
- External rotation
Describe what you are looking/feeling for in each
6
Q
Muscle test- deltoid
A
- Abduct at 90 degrees against resistance
7
Q
Muscle test-supraspinatus
A
Empty can test
8
Q
Muscle test-infraspinatus and teres minor
A
External rotation against resistance
9
Q
Muscle test-subscapularis
A
Lift-off test
10
Q
Muscle test-serratus anterior
A
Flex shoulder at 90 degrees palpating for scapular winging - may perform wall push up
11
Q
Muscle test- rhomboid
A
- Arm on hip, push arm forward at elbow with patient resisting. Watch for scapular winging
12
Q
Neer impingement
A
- Passive forward flexion of the shoulder up to the ear
- If patient has pain, rotator cuff tear or impingement syndrome suspected
13
Q
Hawkins impingement
A
- Shoulder flexed at 90
- Elbow flexed at 90
- Passive internal rotation
- Describe what it indicates: rotator cuff tear or impingement syndrome
14
Q
Crossover test
A
- Passively extended arm over chest
- Describe what it indicates: AC joint arthritis
15
Q
Apprehension test
A
- Pt supine, abduct, and externally rotate the shoulder
- Apprehension due to anterior instability
