  1. What general observations should be made for the shoulder?

A
  1. Redness, swelling, pt willingness to move, deformity, atrophy, scars, attitude of pt, slings, contractors.
  2. A/C, S/C, clavicle, muscle bulk and rounded shoulders.
  3. G/H position: acromion should be aligned w/ the ear. Rounded shoulders, forward head posture.

From the back view, what observations can be made about the scapular position in terms of:
A. Tilting
B. Rotation
C. Protraction and Retraction

A.
Anterior tilt: Inf angle is separated from the thoracic cage (tilting.
Posterior tilt: inf angle is pressed very firmly into the thoracic cage.

B.
Superior: med edge is angled w/ inf border farther away from the midline.
Inferior: Med edge is angled w/ inferior border closer to the midline (can use inclinometer to measure).
Internal: Med edge is separated from the thoracic cage (wigging)
External: Med edge is pressed firmly to thoracic cage.

C. Protraction: med edge is vertical, but farther from midline then normal.
Retraction: med edge is vertical, but closer to the midline then normal.

  1. Head tilt
  2. Asymmetrical Shoulder height
  3. Forward Head
  4. Forward Shoulders

  1. The line through the centre of the head deviates from the midsagittal plane.
  2. Measured by height of acromions or the inf angles of the scapula.
  3. The mastoid process lies anterior to the body of C7
  4. The acromion process lies anterior to the body of C7 or the scapula tilts anteriorly.

How can you screen for scapular movement?

A

Start by having thumbs out at the side. Have pt do abduction of the arms and observe scapular movement.
