What is meant by short bowel syndrome
Shortened bowel due to large surgical resection
So…
- you need ensure adequate absorption of nutrients and fluid
Nutritional deficiencies
- replace vit A, B12, D, E and K essential fatty acids, zinc and selenium , mg
Leads to reduction of hydration from water alone – drink glucose/salt solution
Diarrhoea and high output stomas
-loperamide, and codeine to reduce intestinal motility allowing more absorption
What kind of diet should you have with short bowel syndrome
Should have oral rehydration if needed as well as food high in energy and protein.
Food high in fibre will increase them out put so should be reduced.
Avoid fruits, nuts, seeds, raw veg.
Caffeinated drinks may also increase storm output should have decaf alternative.
Most patient would have a stoma– a surgical created opening on the abdomen that allows bodily waste (urine of feces) to be diverted out of the body
Stoma care
EC/MR capsules not suitable = insufficient effect from drug.
Use forms with quick action: liquids, capsules, uncoated or soluble tablets
Diarrhoea: orbital, magnesium, antacid, iron (ileostomy)
Constipation: opioids, calcium antacids, iron (colonoscopy)
GI irritation plus bleed: aspirin plus NSAIDS
Diuretic/laxatives: dehydration= hypokalaemia
Use k-sparing diuretics or potassium supplements
