Shock Flashcards
define shock
hypoperfusion leading to inadequate substrate delivery, anaerobic metabolism, cellular dysfunction and cell death
what is distributive shock
excessive vasodilation with impaired distribution of blood flow
what type of shock does sepsis cause
distributive and hypovolaemic shock
what is teh cause of obstructive shock
decrease in diastolic filling of the heart leading to a reduction in cardiac output
what are the main causes of obstructive shokc
tension pneumo massive PE cardiac tamponade
treat tension pneumo
needle decompression
2nd ICS midclavicular line via large grey cannula
arrange for chest drain
what are the main causes of hypovolaemic shock
haemorrhage diarrhoea vomitting high output stoma
treat hypovolaemic shock
replace blood with blood
IV tranexamic acid
maor haemorrhage protocol
what is cardiogenic shock
inadequate blood flow due to dysfunction of the heart
treat a regular narrow complex tachy
vagal manouvres
adenosine IV
treat irregular narrow complex tachy
beta blocker or diltiazem
if in HF consider digoxin / amiodarone
treat a regular borad complex tachy
amiodarone
treat an irregular broad complex tachy
AF with BBB – treat as irregular narrow
if preexcited AF – amiodarone
