generate solutions. This step is critical in developing a patient-centered plan of care that promotes the highest level of health and wellness possible for the patient
-component of layer 3 and supports the layer 2 function of refining hypotheses
To generate solutions, the nurse will
-Consider a variety of patient care options as part of the clinical decision-making process.
-Connect previously identified and prioritized patient needs to potential courses of action (the plan of care).
-Consider goals and expected outcomes that are possible for the patient.
-Identify nursing interventions that are appropriate for the individual patient’s unique situation.
The nurse should consider several factors when establishing goals and outcomes, such as
-Best available clinical evidence
-Patient’s values, expectations, and physical condition
-Collaborative approaches to care, including family participation
-Health care team’s clinical expertise
There are two basic tasks the nurse must complete when generating solutions
-Identify expected outcomes using hypotheses
-Define interventions for the expected outcomes
To accomplish these tasks, there are three questions the nurse should ask
What are the desired outcomes for the patient related to the hypotheses, or identified patient problems?
What interventions can help the patient achieve those outcomes?
What interventions are contraindicated and should be avoided to promote safety and prevent further complications?
Nurses should develop goals and outcome statements using
the acronym SMART (specific, measurable, attainable, realistic, timed)
Selection of interventions involves
-Critical-thinking skills to make clinical decisions
-Application of the best evidence for the patient’s situation
-Consideration of individualized patient needs
-Cultural considerations
Nurses consider a variety of factors when selecting interventions:
-Patient safety
-Patient preference
-Etiology, or cause, of the patient problem
-Evidence (scientific literature, standards, policies, guidelines)
-Potential for achievement of the goals and outcomes
-Expertise and competence of health care team members
