- Age (80+ yrs)
- Pre-existing joint disease (RA)
- DM / Immunosuppression
- Chronic renal failure
- Hip / knee joint prosthesis
- IV drug use
What are the clinical features for septic arthritis? (4 things)
- Single hot + swollen joint
- Joint tenderness
- Reduced ROM / X weight bear
- Pyrexia (60%)
(symptoms more subtle in prosthetic joint)
What are some differentials for septic arthritis? (4 things)
- Infection
- Inflammatory arthropathy (gout or pseudo gout in adults / reactive arthritis in kids)
- Post traumatic swelling / bleeding in joint
- Flare up of OA
What investigations should be done for septic arthritis? (4 things)
- Bloods
- Joint aspiration
- Blood cultures
- MRI
What bloods should be done for sus septic arthritis? (5 things)
- FBC
- CRP
- ESR
- Urate levels
- Blood cultures
What is the gold standard imaging technique for septic arthritis?
MRI
Why should you aspirate a joint before starting abx?
Cah you need to have a good sample, if you start abx and then aspirate and culture etc you will find nothing
What should a joint aspirate be sent for? (4 things)
- Gram stain
- Leucocyte count
- Culture
- Polarizing microscopy
What should be the initial management for septic arthritis?
Abx ASAP (make sure after aspiration / culture)
What is the management option for a Native joint with septic arthritis? (2 things)
- Surgical irrigation

What is the management option for a Prosthetic joint with septic arthritis? (2 things)
- Surgical irrigation + debridement
2. Revision surgery
